The first unified album (I won't call it a concept album) is usually considered to be Woody Guthrie's "Dust Bowl Ballads," but I think Merle Travis's "Folk Songs of the Hills" is a little more unified (both came out on shellac).

As for concept albums on vinyl, again country music leads the pack (most notably in chronological order Jean Shepherd, Hank Thompson, Ray Price).
